Social Worker
Advance your career with Franciscan Healthcare as a Social Worker, delivering comprehensive support within a collaborative care team. The Social Worker delivers comprehensive psychological assessments, diagnostic, therapeutic interventions, case management, and resource referrals for individuals, couples, families, and groups, as appropriate, anywhere within the environment of Franciscan Healthcare.
Hours: Full-time, minimum 35 hours per week; exempt
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
• Patient advocate with physician and other health care providers.
• Gathers and evaluates patient information through interviews, assessments, and observations to identify needs, determine appropriate interventions or referrals, monitor treatment effectiveness, and recommend adjustments to care or services
• Develop, organize, implement, and evaluate the social work program in accordance with acceptable social work practice, the National Association of Social Worker’s Code of Ethics, and Franciscan Healthcare policies and procedures.
• Collaborates and partners with hospital patient care coordinator to provide discharge planning as well as performance of swing bed intake coordination and assessments.
• Serves as a resource for Franciscan Healthcare and the community regarding advanced directive planning and communication.
• Promotes/maintains positive interdepartmental, community and referral relations.QUALIFICATIONS:
Education and/or Experience
• Bachelor’s Degree from a social work program approved by the Council of Social Work Education; Master’s in Social Work preferred.
• Current license in the State of Nebraska required.
• Previous social work experience in a medical setting desired.Other Competencies

• Skills in interpersonal relationships, clinical assessment, group process and high levels of verbal and written communication.
• Ability to interact with other professionals as part of a multidisciplinary team.

• Assimilates new clinical knowledge and skill gained from conferences, workshops, seminars or professional literature into practice.
• Ability to contain emotional control and have knowledge and use of effective coping mechanisms when handling emotional situations.
