It's that time of year again! Schedule your School Physical NOW by calling Centralized Scheduling at 402.372.2404. The downloadable packet below is for students entering grades 7-12. It is recommended you fill out these forms prior to your appointment and bring them in with you.
PLEASE NOTE: A parent/guardian MUST accompany your student to the appointment OR complete the online registration. Urine samples are no longer needed for the School Physical.
